The “Form 12 B” page appears. The system displays the following.

Employment Unit

The employment unit to which you are mapped.

Date format

The date format as applicable to the login user

Employee Code

The unique code identifying an employee.

Employee Name

The name of the employee.

Tax Year

The tax year selected in the previous page.

To record previous employment details

Employer’s Name

The name of the employer

Employer’s TAN

The tax account number (TAN) of the employer

Employer’s PAN

The permanent account number (PAN) of the employer

Employment From

The date from which the employee has joined.

Employment To

The date until which the employee has separated.

Section 10 Allowances

The allowances deducted under section 10 by the employer

Perquisites

The perquisite value given by the employer

Total Salary (Including Allw. And Perq)

The total salary of the employee.

Professional Tax Deducted

The professional tax deducted by the employer.

Provident Fund

The value contributed towards provident fund

LIC Premium

The premium paid towards LIC policy.

Income Tax Deducted

The income tax deducted by the employer for the tax year.

Remarks
