The name of the employee against whom the disciplinary action was initiated.

The number of the employee’s assignment.

Disciplinary Action No.

The unique disciplinary action number generated by the system when the disciplinary action record was submitted.

Disciplinary Action Type

The type of disciplinary action type can be one of the user-defined disciplinary action type.

Disciplinary Action Status

The status of the disciplinary action can be one of the user-defined status.

Submission Date

The date on which the disciplinary action was submitted

Disciplinary Action Initiated by

The name of the employee who initiated the disciplinary action

Reason

The reason for taking disciplinary action. For example, theft/absenteeism etc.

Action Details

The details of the disciplinary action taken.

For example, the action taken can be suspension of the employee for a period of 1 month.
