The “Enroll COBRA Acceptance” page appears.

Benefits Enrollment Unit |
The name of the organization unit in which the benefits enrolment unit has been deployed. If there is more than one value, the system displays “Select” as the default value in the drop-down list box. From the “Benefits Enrollment Unit” drop-down list box, select the organization unit for which the enrollment of COBRA acceptance is to be performed. Tell me more. |
Enter the code of the employee in the “Employee” who has to be enrolled in COBRA acceptance plan.
The employee’s SSN number will get displayed in the “SSN Number” field.

Enter or select the following.
Program |
From the drop-down list box, select the program for which you need to record the COBRA acceptance. The drop-down list box displays the list of all the program short descriptions defined in the “Create Benefit Program” activity of Benefits Definitions and interact with the selected benefits enrollment activity. |
Qualifying Event |
From the drop-down list box, select the event in which you want to enroll in COBRA. The drop-down list box displays the list of the events defined in the HRMS General Masters component. By default, the system displays “Select” in the drop-down list box. If there is only one value, the system will default with the value. |
Participation From Date |
The date from which the employee can enroll in a plan. |
Select the “Get Plan” pushbutton to get the list of plans that are mapped to the selected program for which Cobra acceptance is permitted and from the specified participation date.
The system displays the following details for the selected benefit program in the multiline.

The unique code that identifies the plan type. |

Participation From Date |
The date from which the plan is open for enrollment. |
Participation to Date |
The date until which the plan is open for enrollment |
Acceptance Date |
The date from which the employee is accepted in Cobra enabled plans. |
Enter the date from which the employee is enrolled in the plan by entering it in the corresponding “Acceptance Date” column of the multiline.

The date from which the employee is accepted in Cobra enabled plans. |
Remarks |
Enter the remarks, comments, and opinions if any. |
Note that you must enter the “Acceptance Date” for each plan selected.
Select the “Save” pushbutton to save the details entered and enrolls the employee in the plan(s).
