The system displays the following.
Date format |
The date format of the previous page. |
Benefits Enrollment Unit |
The benefits enrollment unit selected in the previous page. |
Employee |
The code and name of the employee. |
SSN |
The social security number of the employee |
Plan Code |
The unique code that identifies an employee |
Plan |
The description of the plan. |
Plan Category |
The category to which the plan is mapped. |
Premium Frequency |
The number of times the premium is to be paid. |
Currency |
The currency in which the premium is paid. |
Event |
The event selected in the previous page. |
Coverage Level |
The coverage level option selected at the time of enrollment. |
Coverage From Date |
The date from which the employee is covered in the plan. |
Coverage To Date |
The date until which the employee is covered in the plan |
The system displays the following details if “Coverage Details” option is selected in the “Coverage Level” drop-down list box in the “Create Benefit Plan” activity for the plan. The system will display either in terms of amount or in times pay.
In Amount |
The amount to be paid for enrolling under a plan. |
In Times Pay |
The amount to be paid in times pay for enrolling under a plan. |
Minimum |
The minimum amount to be paid for enrolling under a plan. |
Maximum |
The maximum amount allowed for enrolling under a plan |
Incremental Steps |
The percentage increase in the amount of contribution. |
GIA |
The guaranteed issued amount defined by the provider organization for a benefit plan. |
Self Coverage |
The value to be covered under the plan. |
The system displays the following in the multiline.
Family Member Code |
The unique code that identifies a family member. for example, 01 can indicate mother and 02 father etc. |
Dependents |
The name of the dependent |
Relationship |
The relationship of the dependent with the employee |
Minimum Amount |
The minimum amount that can be covered. |
Maximum Amount |
The maximum amount that can be covered under the plan. |
Incremental Steps |
The percentage increase in the amount of contribution |
Coverage Amount |
The value to be covered under the plan for the corresponding dependent. |
The system will display the following in the “Contributions” group box.
Employee Contribution |
The amount to be contributed by the employee for the corresponding coverage value. |
Employer Contribution |
The amount that will be contributed by the employer for the corresponding coverage value. |
The system also displays the remarks, opinions, etc., that have been entered at the time of enrolling in the plan.
You can also enter opinions, comments and other details if any, in the “Authorizer's Remarks” field.
Select the “Authorize” pushbutton to authorize all the selected records.
Select the “Unauthorize” pushbutton to unauthorize the selected records.
Select the “Reject” pushbutton to reject the enrollment of the employee in the plan.
