You can use this page to record COBRA acceptance
Select the “Record Cobra Acceptance” hyperlink from the left pane.
The “Record COBRA Acceptance” page appears.
The system displays the following.
Date format |
The date format applicable to the login user. More Details. |
Employee |
The code and name of the employee. |
SSN |
The social security number of the employee. |
Enter or select the following.
Program |
From the drop-down list box, select the program for which you need to record the COBRA acceptance. The drop-down list box displays the list of all the program short descriptions defined in the “Create Benefit Program” activity of Benefits Definitions. |
Qualifying Event |
From the drop-down list box, select the event in which you want to enroll in COBRA. The drop-down list box displays the list of the events defined in the HRMS General Masters component. By default, the system displays “Select” in the drop-down list box. If there is only one value, the system will default with the value. |
Participation From Date |
The date for enrollment. |
Select the “Get Plans” pushbutton to get the list of plans that are mapped to the selected program and for which Cobra acceptance is permitted and the enrollment in the plan has been done previously.
The system displays the following details for the selected benefit program in the multiline.
Plan Category |
The plan category to which the plan is mapped. One amongst the five are displayed namely “Health", "Life & AD\D", "Disability", "FSA" and "Savings" |
Plan Code |
The unique code that identifies the plan. |
Plan Description |
The description of the plan. |
Participation From Date |
The date from which the plan is open for enrollment. |
Participation to Date |
The date until which the plan is open for enrollment |
Acceptance Date |
The date from which the employee is accepted in Cobra enabled plans. |
Enter the opinions, comments and other details if any in the “Remarks” field in the multiline.
Select the plans for which you want to enroll under the COBRA act by checking the corresponding records in the “Select” column of the multiline.
Note that you must enter the “Acceptance Date” for each plan selected.
Select the “Save” pushbutton to save the details entered.
