Date Format |
The date format applicable to the login user. More details |
Employee |
The name of the employee selected in the previous page. |
SSN |
The social security number of the employee. |
Program |
The name of the program to which the plan is mapped. |
Currency |
The currency in which the premium paid. |
Plan Type |
The type of the plan. |
Plan Description |
The description of the plan. |
Coverage From Date |
The date from which the employee is covered under the plan. |
Coverage To Date |
The date until which the employee is covered under the plan |
The system displays the following in the multiline.
Family member Code |
The unique code that identifies a family member. For example, 01 can indicate mother and 02 father etc |
Dependents |
The number of dependents allowed to be covered under the plan. |
Relationship |
The relationship of the dependent with the employee. |
Enroll |
Select the check box to indicate that the dependents are enrolled in the plan. If the dependents have been enrolled earlier, the system will display as checked in the Enroll check box |
Select the “Save” pushbutton to save the details entered.
