Tab: Carrier (Provider) Information

Carrier Name

The name of the carrier or the provider.

Address

The address of the carrier.

City

From the dropdown, select the city where the carrier is located.

State

From the dropdown, select the state where the carrier is located.

Country

From the dropdown, select the country where the carrier is located.

Zip

The zip code of the place.

Phone

The phone number of the carrier.

Mobile

The mobile number of the carrier.

Email

The email ID of the carrier.

Website

The official website of the carrier.

Tab: Dependent Coverage Options

Coverage Level Code

The code that identifies the coverage level.

Coverage Level Description

The description of the coverage level. For Ex: “Employee”, “Employee + Spouse” and “Employee + Spouse + Children”.

Max. No. Of Dependents

The maximum number of dependents allowed in the coverage level.

Max. Child Age

The maximum child age covered in the plan.

Note: If Maximum Child Age is not entered, the system defaults the maximum child age as 26.

Relationship Settings

The relationship settings of the coverage level.

Click icon on any row to view the relationships which are eligible in that coverage level.

Relationship

The relationship that is eligible under the coverage level.

No Of Dependent

The number of dependents eligible in the specific relationship. Ex: If two children are eligible, key in 2 against child.

Applicable

Select the check box to indicate that the dependents are eligible under the plan.

Tab: Plan Types

Plan Category

From the dropdown, select the category to which the plan is mapped. The dropdown displays the predefined plan categories “Health", "Insurance", "Flexible Spending", "Savings" and “Health Savings”.

Plan Types

The description of the selected plan type.

Tab: Parameters

Parameter Type

This is a user configurable group which helps to group a set of employees with common work conditions or attributes. The dropdown has predefined values like “Age Group", "Employee Group", "Salary Group", and “Service Group”. The purpose of this group is to define common premium rates, coverage amounts and limits. Example – A particular health plan has different premium rates for Union employees – we could have a Union group and define the premiums for that group.

If the Parameter Type is “Age Group”, “Salary Group” and “Service Group”,

Group Name

The name of the benefit group. User could give any name to the group which could be easy for administration. Example – Highly Compensated Group with a Salary range. FLSA Exempt Group, Non Exempt Group

From & To

The group range can be given in From and To. For Example: If there are some benefits applicable for employees who are above 50 years of age, you could create a Age Group called “Above 50 years” and give the from and to age range

Similarly any Salary group can be created with the salary range From and To.

Service group can be created with a service From and To range.

 

 

If the Parameter Type is “Employee Group”,

Group Name

The name of the benefit group. Employee group is a combination of multiple attributes like Org Unit, location, gender, FLSA status, Tobacco user etc

Org Unit

From the dropdown, select the organization unit applicable.

Work Location

From the dropdown, select the work location applicable.

Grade Set

From the dropdown, select the Grade set applicable.

Gender

From the dropdown, select the gender applicable.

Union

From the dropdown, select the union of the employee.

Employee Type

From the dropdown, select the employee type applicable.

Benefit Group

From the dropdown, select the benefit group if there is any specific benefit group in the organization.

FLSA

From the dropdown, select an option to specify whether FLSA Exempt or Non Exempt

Tobacco User

From the dropdown, select “Yes” and “No”.

Note: All dropdowns have option “All”. System defaults ‘All’ if there is no specific attribute required.
