Adding Risk Questionnaire Response

Page Overview:

Select the “Add Responses” link at the bottom of the “Maintain Risk Assessment Questionnaire” screen.

The “Risk Questionnaire Response” page appears.

The system displays the following field in the header section:

Questionnaire#/Rev#

The unique identifier of the questionnaire and the revision # as selected in the previous page.

Question ID

The unique identifier of the question from the questionnaire.

The system displays the following field:

Question

The entire text of the question.

Enter the following details for each of the responses to the question in the “Responses” multiline:

Question ID

The unique identifier of the question.

The system displays the following field in the multiline:

Question

The entire text of the question.

Response	The pre-defined response to the question.
Score	The score value for the response to the question.
Severity	The severity attached to the score value for the response to the question.
Mitigation Comments	Additional information on the action of reducing severity and seriousness of the potential risk associated with the response.
Post Mitigation Score	The score value for the response to the question after the mitigation actions are taken against the risk addressed by the question..
