How to use the web page

Using online help

OSHA Unit

From the drop-down list box, select the required option. The combo is defaulted with the login OU for you to choose.

Establishment

From the drop-down list box, select the required Establishment code.

Note: On clicking “Save”, the establishment details get saved and establishment code gets generated in this field.

Establishment Code

The establishment code. Ideally this will be the work location.

Establishment Name

The name of the establishment.

Default Establishment

From the drop-down list box, select the required default establishment. The combo is loaded with “Yes” and “No” options for you to choose. By default, “Select” is displayed on launch of the page.

Parent Establishment

From the drop-down list box, select the required parent establishment. The combo is loaded with “Yes” and “No” options for you to choose. By default, “Select” is displayed on launch of the page.

Industry Description

The description of the industry, for example, Manufacturing, Retail, Baking etc.

Address Line 1, 2, 3

The address line 1, 2 and 3

Country

From the drop-down list box, select the required country. The combo is loaded with various countries for you to choose.

State

From the drop-down list box, select the required state. The combo is loaded with various states for you to choose.

City

From the drop-down list box, select the required city. The combo is loaded with various cities for you to choose.

Zip

The zip code of the city in numeric format (in five digits).

Phone

Enter the phone number in this field.

Email ID

Enter the email id in this field.

Identifier Type

From the drop-down list box, select the type of identifier. The combo is loaded with various options for you to choose. By default “Select” is displayed on launch of the page. Mandatory.

Value

Enter a numeric value in this field. Mandatory.

Note: Enter a unique nine digit numeric value, on selecting “Employer Identification Number” in the “Identifier Type” drop-down.

Remarks

Key in any remarks or feedback, if any.

Administrator For

From the drop-down list box, select the required administrator. The combo is loaded with various options for you to choose. By default “Select” is displayed on launch of the page. Mandatory.

Primary Admin

Enter a valid employee code as primary administrator in this field. Mandatory.

Help facility available

The following details are displayed on entering the “Primary Admin” field.

Primary Admin Name

The name of the primary administrator.

Position

The position of the primary administrator.

Address

The address of the primary administrator.

Alternate Admin

Enter a valid employee code as alternate administrator in this field. Mandatory.

The following details are displayed on entering the “Alternate Admin” field.

Alternate Admin Name

The name of the alternate administrator.

Position

The position of the alternate administrator.

Address

The address of the alternate administrator.

Effective From

The effective from date. The Establishment detail recorded will be effective from this date entered here. Mandatory.

Effective To

The effective to date.
