Date Format

The date format applicable to the login user. More Details

Trainee Code

The identification number of the employee participating in the course.

Trainee Name 

The name of the employee who has completed the course.

Course

The name of the course selected in the previous page.

Assignment No.

The assignment related to the course.

Training No.

The training number of the course.

Start Date

The start date of the course. 

End Date

The end date of the course.

Feedback Parameter

The feedback parameter that has been mapped to that particular course. This feedback parameter has values fetched to the sub feedback parameters available, like for instance - Rating of Program, Effectiveness of faculty etc.

Rating

Select the appropriate rating for the employee from the  drop- down list box.  Since it has more than one value, by default, the system displays a blank value in the drop- down list box.  Select the required value from the drop- down list box.  For example, it can take the values: “excellent”, “good”, “fair” and “bad”.

Remarks

The remarks pertaining to the feedback.

Faculty

The name of the faculty for whom you are recording feedback.

Parameter

The parameter for which you are recording the feedback.

 

Rating

Select the appropriate rating for the employee from the  drop- down list box.  Since it has more than one value, by default, the system displays a blank value in the drop- down list box.  Select the required value from the drop- down list box.  For example, it can take the values: “excellent”, “good”, “fair” and “bad”.

The remarks pertaining to the feedback.

Did you find the program useful?

You can select the check box:

  • “Yes” to indicate your positive opinion.

  • “No” to indicate your negative opinion.

How did you find the course duration?

You can select the check box:

  • “Too Short” to indicate the period of training is inadequate.

  •  “Just Right” to indicate the period of training is appropriate.

  •  “Too Long” to indicate the period of training is unnecessarily long.

Should it be recommended for your fellow colleagues to attend?

You can select the check box:

  • “Yes” to indicate your positive opinion.

  • “No” to indicate your negative opinion.

To save the record

Select “Save” pushbutton to save the details entered.
