Voluntary Self Identification
Select “Voluntary Self Identification” under “Employee landing page”.
The “Voluntary Self Identification” page appears.
The following details are displayed in the header:
Employment Unit |
The employment unit. |
Employee ID |
The id of the employee. |
Employee Name |
The name of the employee. |
SSN |
The Social Security Number of the employee. It is a unique citizenship identification number for an employee. |
Joining Date |
The date of joining the company. |
Employee Type |
The type of employee as to whether “Regular Full-time” or “Part-time”. |
Enter the following field details under “Ethnicity and Race” section:
Gender |
From the drop-down list box, select the gender. The combo is loaded with various options for you to choose. The drop-down is defaulted with “Select” on launch of the page. Note: You also have the option not to disclose. |
Ethnicity |
From the drop-down list box, select the ethnicity. The combo is loaded with various options for you to choose. The drop-down is defaulted with “Select” on launch of the page. Note: You also have the option not to disclose. |
Race |
From the drop-down list box, select the race. The combo is loaded with various options for you to choose. The drop-down is defaulted with “Select” on launch of the page. Note: You also have the option not to disclose. |
Click the hyperlink “What are the Race/Ethnic Definitions?” to view the definitions of different race in PDF format.
Enter the following field details under “Protected Veteran Status” section:
Veteran Status |
From the drop-down list box, select the required status. The combo is loaded with various options for you to choose. The drop-down is defaulted with “Select” on launch of the page. Note: You also have the option not to disclose. |
Discharge Date |
Enter the discharge date, if required. |
Click the hyperlink “What is a Protected Veteran Status?” to view the details of protected veteran status in PDF format.
Enter the following field details under “Section 503” section:
Disability Status |
From the drop-down list box, select the required disability status. The combo is loaded with various options for you to choose. The drop-down is defaulted with “Select” on launch of the page. Note: You also have the option not to disclose. |
Disability Learned Date |
Enter the disability learned date, if applicable. |
Select the check box “Yes” next to “Need Reasonable Accommodation” if any reasonable accommodation is needed otherwise click “No”.
