Employee

The name of the employee selected in the previous page.

SSN

The social security number of the employee.

Plan Category

The category to which the plan is mapped. 

Currency

The currency in which the premium paid.

Plan code

The unique code to identify the plan. 

Plan

The description of the plan.

Coverage From Date

The date from which the employee is covered under the plan.

Coverage To Date

The date until which the employee is covered under the plan

Family member Code

The unique code that identifies a family member. For example, 01 can indicate mother and 02 father etc

Dependents

The name of the dependent.

Relationship

The relationship of the dependent with the employee.

Enroll

Select the check box to indicate that the dependents are enrolled under the plan. If the dependents have been enrolled earlier, the check box remains selected.
